Pathways Telehealth Nurse Service

CCDR developed the Pathways telehealth nurse program. It is a free service that anyone can access, regardless of their location. The program provides a service for children and adults and covers a wide range of conditions from rare and genetic disease, familial conditions, oncology, neurological, developmental, cardiac, dermatology, metabolic and complex chronic conditions. For people searching for a diagnosis, we have a special service to help them with that as well.

Pathways nurses conduct a holistic consultation to understand everything that is happening with the patient in front of them. It’s a model of care that helps people navigate the complex health system and access all that is available to them. The Pathways nurse then works with the patient to develop a care plan and offers support and guidance along the way.
